Dr. Bautista brings over 30 years of medical experience, including nearly 20 years in family practice. He earned his bachelor’s degree from Philippine Union College and his Doctor of Medicine (MD) degree from Far Eastern University, both in the Philippines. He later completed his PhD in Biochemistry at Loma Linda University in California.

Crisanto Reyes is a Board Certified Internal Medicine Specialist. Dr. Reyes completed Residency Programs in Chicago, Illinois at both Oak Forest and Cook County Hospitals. He also served an Internship at Cook County Hospital.
Dr. Reyes has been a member of the Iroquois Memorial Hospital staff since 2004 and has been practicing medicine for over 25 years. He sees patients of all ages at IMH Medical Group - Gilman and IMH Medical Group - Milford.

Susan Hanson is a professional Family Nurse Practitioner with experience in the clinic, convenient care, and long-term care environments. She provides a patient-centered approach, focused on helping those with simple needs to those with complex medical and psychological needs. Susan has a special interest in working with the diabetic patient.
