What is Swing Bed?

When transitional care is needed to fill the gap between a hospital stay and home, a swing bed at IMH provides a private setting to get the necessary care you need while staying in the comfort of your own community. 

Why Choose the Swing Bed Program at IMH?

The IMH Swing Bed Program provides patients with a private room within the hospital to serve as an alternative to a nursing home or rehab facility stay after an illness, surgery, or other procedure.

During your stay, you can count on our skilled staff to help you meet your goals with personalized therapy and educational plans to regain the strength and confidence you need to return home. Our Hospitalists will provide you with consistent 24/7 healthcare, creating a patient - provider relationship for the duration of your stay. Once you are discharged, you can choose to return to your primary care provider in your own town, or if you do not have a primary care provider, our Case Manager will work with you to establish a provider to continue your rountine care at one at one of our five IMH Medical Group Clinics.

Qualifications and Cost
How do I qualify for the program?
  • A referral for the Swing Bed Program can be made for a patient who receives Medicare, Medicare Advantage, or has private insurance coverage 

  • Patients with traditional Medicare Advantage must have 3 inpatient midnight stays within a 30 day period, and replacement plans require PA

  • The referral must be generated from a physician’s order

What is the program cost?

Medicare

Days 1-20: 100% coverage
Days 21-100: 80% coverage
Days 101+: No coverage

Medicaid Coverage

Not covered in the state of Illinois.

Medicare Advantage Plan

Coverage and length of stay in the program will be determined once the physician's order has been authorized.

Private Insurance Coverage

Available with some insurance plans. Coverage and length of stay in the program will be determined once the physician's order has been authorized.
